This rock is a beautiful mish mash of many minerals.  It likely was a layered rock that was metamorphosed and then cracked and then filled in with quartz.

Chemical composition: It is difficult to know the composition of this specimen.  It is likely the white mineral is quartz.  The green is likely serpentine or oceanic basalt.
